Four Ways to Register
1. Fax form with credit card information to (212) 594-6717.
2. Mail form with your check or credit card information.
3. Use MS Word to complete the registration form and email.
4. Submit online form (see below).
Please call the Embassy Suites at 1-877-371-2125 to make hotel reservations. Mention our group name-CDVCA and our group code-G33 to receive the conference rate. All rooms are suites that can accommodate up to 4 people. Reservations MUST made by February 26, 2008.

Conference (non-members: $695.00; members: $620.00)
Both Pre-Conference Workshop & Conference (non-members $875.00; members $775.00)
Important notes if paying by credit card:
* We only accept VISA or MASTERCARD
* Provide credit card #, expiration date, name as it appears on card
* We do not need the credit card billing address
* A receipt will automatically be mailed when charge is processed
* If credit card holder is different from the registrant, please provide requested contact information for the person who will attend the event
* Early-bird discount and member rate will be applied before the credit card charge is processed. Only the regular Non-Member rate will appear in the online form. Membership discount will be applied upon verification of membership status.
